Physician Advice on E-Cigarettes for Smoking Cessation: Differentiating Evidence From Values

Physicians often face a genuine dilemma when it comes to counseling patients who smoke about e-cigarettes. Although most support the concept of harm reduction in other spheres, “tobacco harm reduction” is hotly debated.Central to these debates is the potential of nicotine e-cigarettes to help people quit smoking. In this issue of JAMA, Leavens and colleagues move this debate forward by presenting recommendations for US-based clinicians about nicotine e-cigarettes for smoking cessation. Developed by a working group within the international Society for Research on Nicotine and Tobacco, the recommendations propose that e-cigarettes be integrated into conversations on the risks and benefits of all evidence-based pharmacologic treatments for quitting smoking, given that nicotine e-cigarettes are more effective for smoking cessation than US Food and Drug Administration (FDA)–approved nicotine replacement therapies (NRTs).
