In response to rising colorectal cancer incidence rates among adults younger than 50 years, in 2021, the US Preventive Services Task Force (USPSTF) updated its guidelines to recommend that average-risk adults begin colorectal cancer screening at age 45 years. Other groups have made similar recommendations. However, there are few reports on screening colonoscopy neoplasia yields among younger adults following the USPSTF guideline change. We investigated colorectal neoplasia yields at screening colonoscopy among adults aged 45 to 49 years vs 50 to 54 years who were members of Kaiser Permanente Northern California (KPNC), a large integrated health system that provides comprehensive inpatient and outpatient services to more than 4.5 million Northern California members.
