Frontline chemotherapy-free induction for mantle cell lymphoma
Mené sur 131 patients atteints d'un lymphome à cellules du manteau et âgés de moins de 65 ans (âge médian : 56 ans), cet essai de phase II évalue l'efficacité, du point de vue du taux de réponse globale, et la toxicité d'un traitement d'induction combinant ibrutinib et rituximab dispensé avant une chimio-immunothérapie de première ligne de type R-HCVAD (rituximab plus cyclophosphamide, vincristine, doxorubicine et dexaméthasone administrés de façon hyper fractionnée)
Standard treatment for young patients with mantle cell lymphoma involves intensiveregimens (rituximab–anthracycline–cytarabine-based) with or without autologous haematopoieticstem cell transplantation (HSCT) and rituximab maintenance and yields high completeresponse rates (61–89%). However, relapses still occur, with 3-year progression-free survival rates of 60–85%,and such treatment regimens are also associated with non-negligible adverse events.
