Adults with high-risk acute lymphoblastic leukaemia are recommended for intensive treatment with contemporary protocols. Patients with Philadelphia chromosome-positive or BCR-ABL1-positive Philadelphia chromosome-positive acute lymphoblastic leukaemia, who constitute about 25% of patients with B-lineage acute lymphoblastic leukaemia, traditionally have a very poor outcome with chemotherapy, particularly if they did not undergo allogeneic stem-cell transplantation in first remission.
